Colonoscopy (diagnostic)
Reviewed by Dr. Neel Patel · data refreshed June 10, 2026 CPT 45378
A diagnostic colonoscopy (CPT 45378) is a routine, well-standardized procedure — but the bill is really three or four bills: the facility, the anesthesia, the pathology lab if tissue is examined, and the physician. The price below is the facility portion only.
This is the hospital (facility) fee. Anesthesia and pathology (if tissue is examined) usually bill separately. Always confirm with the hospital.
What you're actually paying for
One procedure usually means several bills. Here's the episode, in order:
- 01
Pre-procedure visit and bowel prep
Billed separatelyThe consult bills separately; the prep kit is usually a pharmacy cost.
- 02
The procedure (facility fee)

- 03
Anesthesia or sedation
Billed separatelyUsually a separate bill from the anesthesia group.
- 04
Pathology, if a polyp or tissue sample is taken
Billed separatelyLab examination of removed tissue bills on its own — and polyp removal also changes the procedure code (see CPT 45385).

Questions worth asking before you book
- If a polyp is found and removed during my colonoscopy, how does that change my bill?
- Is anesthesia included in this quote? Is the anesthesia group in my insurance network?
- I'm scheduled as a screening — if the procedure becomes diagnostic, what happens to my cost-sharing?
- If I'm paying cash, can I have a single all-in Good Faith Estimate covering facility, anesthesia, and pathology?
